Long Term, Extended Care

Lost Rivers Medical Center offers skilled nursing, extended care to those unable to care for themselves; provided they meet the qualifications of the program. 

Common Questions


Our team is focused on helping you maintain your independence. As a Long Term Swing Bed patient, you’ll be encouraged to do as much as you can for yourself.

Doctor Visits

The doctor will evaluate your medical status monthly and be available if a need arises.


We expect you to wear your own clothes.  Health Care Services Group will provide laundry service.

Personal Items

You may furnish your own personal items. Small items may be brought from home for your comfort. Hospital Staff will check all electrical items brought in for safety.


We encourage you to take part in a variety of daily activities to enhance and help you maintain your independence. You will be given an activities calendar and a list of available activities upon admission to the program. If there are other activities that you enjoy, they may be brought in from home. Please talk with a member of your care team before you bring them in.

Qualifications Include:

  • Your doctor has determined a need for daily skilled care.
  • You are willing to participate in the daily skilled care program.
  • You must complete an initial mental health and social history screening by Social Services or Designee.

Lost Rivers Medical Center is not equipped with a locked memory care unit.  For patient safety we cannot accept Long Term patients that wander and have deficient cognition that impairs ability to make safe mobility choices.

Payment Information

Lost Rivers Medical Center accepts payment from private funds and Medicaid. Lost Rivers Medical Center will assist the resident/guardian with obtaining reimbursement from Medicaid.

Medicaid funding may be available to residents when their resources meet the State guidelines.

Self-Pay Residents are responsible for their room fees one month in advance.

For services, other than, the room and board, the patients current medical insurance will be billed. (Medicare, Medicaid, supplement, etc.)
